I have one. I haven't tried using it on an iPhone (since I don't have one), but I did use it a few times to catch a game at work :-) It worked OK.
Keep in mind that while you are using the Slingbox, whatever cable device you have it hooked it up to will be tied up. So if you have your Slingbox in the den and decide to watch a game, anyone else in the den is going to be watching MLB too.
You also want to have decent upload speed at home... I have DSL and it was doing OK, cable modem would probably be much better.
Woot.com used to have sales of these every few weeks but I haven't seen them in a while.

Consider getting Hulu Plus for about $8/mo. I have that specifically so I watch certain shows when I'm at the gym on the elliptical machine, or whatever. I have a Droid X on Verizon's unlimited data plan and rarely have problems with buffering.
I also use HBO GO (free for HBO customers) and Netflix on the phone with great results.
I don't think any of them would allow you to watch MLB though.
